Camper Forms & Letters

This is a pdf of the camper application which can be sent to families who do not have access to computer.  This should only be distributed as a hard copy document, by request, and should not be posted on chapter websites.  Be sure to fill out the cover page so the family knows where to send the app, and most importantly, be sure to enter all camper information into Active using the ‘Add/Register New Entry’ button on your campus’s Active homepage.
This is an acceptance letter to send after a camper has been approved by your PD.  You can copy/paste and email it to the camper parent, and be sure to edit the yellow highlighted sections!  If you would like to add any information particular to your camp, you are free to do so, but please do not remove any information.  WHEN YOU SEND THE EMAIL, MAKE SURE YOU ATTACH THE “Camper HealthCare Recommendations” FORM AS WELL. 
A PDF that must be filled out and signed by the camper’s doctor and returned to your chapter via email, fax, or snail mail.  For more info on this form, check out the Parent Acceptance Letter which includes instructions. 
ONLY SEND TO FAMILIES WHO ALSO COMPLETED A HARD COPY CAMPER APPLICATION.  99% of families will fill this out online after they receive their acceptance letter and do not need the hard copy.  For more information, see the Parent Acceptance Letter.
A form anyone applying to be a CIT (counselor in training) must get filled out as a letter of recommendation.
 

Camp Forms & Letters

A form for Student Support coordinators and Co-Chairs to review everyone who applied to be a counselor.
A series of questions to be filled out by co-chairs and sent to camp advisor prior to camp, giving an overview of your camp traditions and expectations for the week.
A letter of intent for professional staff to sign, committing to volunteering for the week of camp.

An incident report to be filled out by camp advisor and unit leader/counselor for camper behavior issues at camp that gets turned in to your program director after camp.
A plan to fill out with camper who had a behavioral incident to discuss the problem, reasoning, and further actions.
An incident report to be filled out by the camp nurse for any medical issues at camp that gets turned in to your program director after camp.
This document is for your nurse to fill out for every camper to keep track of medications received during camp.
This document is for your nurse to fill out for campers who take one-time/as needed medications, or if a dose of their medication was omitted.
